COBRA Election

The person who wants to purchase continued coverage must:
  • Complete the Continuation of Coverage (COBRA) election form within 60 days from the later of:
    • Termination of normal coverage under the plan, or
    • Receipt of the COBRA notice and election form.
  • Mail the completed form to the address noted on the application, with a postmark, within the 60-day period.

The COBRA participant must pay the required monthly costs for the continuation of coverage.
