The monthly cost must be paid by the covered person to the Plan Administrator by the first of each month, without waiting for a bill or other notice.

Costco may subsidize a portion of the monthly COBRA cost if:

You are on approved LOA for your personal medical condition that extends past the maximum period of benefits and you elect to continue your COBRA health care coverage at your own expense, Costco will assist you with your monthly COBRA payments and continue your benefits according to the following schedule for a maximum of six months:
