COBRA Special Provision

In the event benefit plan coverage is terminated as a result of the employee’s death or extended medical leave of absence due to the employee’s terminal illness, Costco will reimburse COBRA premiums according to the schedule below:
  • Non-work related death - One year for covered spouse and children if no other health coverage is available.
  • Work-related death - Three years for covered spouse and children, after which coverage may continue on a contributory basis as follows:
    • Dependent Children: Until age 19, provided no other health coverage is available.
    • Spouse: If coverage is continued for dependent children, spouse can remain covered until all covered children have reached age 19, provided no other health coverage is available.
  • Exhausted medical leave of absence (including Family Medical Leave) due to terminal illness - Up to 18 months for employee and any covered dependents if no other health coverage is available when the following requirement is met: Must be diagnosed as terminally ill with a life expectancy of two years or less as certified by a life insurance carrier approved by Costco in accordance with the Accelerated Life Benefit.

Note: The spouse and children must meet COBRA eligibility requirements to be eligible for this benefit.
