Choice Plus Plan

The Choice Plus plan is a point-of-service plan that gives you and your family access to Aetna's network of providers when you need care. Under this plan, you are not required to choose a Primary Care Physician (PCP); however the most effective access to care is still achieved by selecting and using a PCP. In addition, this plan does not require you to have a referral to seek specialty care.

Costco offers two Choice Plus plans: the Choice Plus plan and the Part-Time Choice Plus  plan. The plans provide the same coverage but differ in how they pay benefits. See the Summary of Benefits chart for a comparison of the two Choice Plus plans.

Receiving care: When you need care, you decide whether to go to an in-network or out-of-network provider. When you receive in-network care, the plan pays benefits at a higher level (once you've met the annual deductible), so you pay less out of your own pocket. You'll also avoid claim paperwork because your in-network provider will file claims for you. When you use an out-of-network provider, your out-of-pocket costs will be higher – the plan reimburses you less and you may also be responsible for any amount that is above what Aetna considers to be reasonable and customary for that service. 

The Aexcel network: Remember, if you enroll in a Choice Plus plan, you'll save when you use a specialist who belongs to the Aexcel specialist network, see the Aexcel section for details. Please note that the Aexcel specialist network is only available in certain locations. Check DocFind for network availability in your area.

Sharing the cost: You will pay a copay for a Doctor’s office visit. For all other services, you must meet a deductible each year before the plan starts to pay benefits. . After you meet the deductible, the plan pays part of your covered expenses, and you pay part – this is called your coinsurance.  There is a limit to how much you must pay for coinsurance each year.  Once you have reached your coinsurance annual maximum, you will not have to pay anymore coinsurance for the rest of the calendar year.     

Summary of benefits: The Summary of Benefits chart shows how you and the plan share the cost of covered services, including, copays, deductibles and coinsurance amounts.
