Declining Medical Coverage |

You may choose not to participate in Costco medical and dental programs. To decline coverage you must first access the enrollment system and elect to decline coverage. You then must complete a Decline Coverage Acknowledgement form and submit it to the Employee Benefit Department before your request to decline coverage can be processed.
You may submit your Decline Coverage Acknowledgment form in any of the following ways:
- By intercompany mail to: Location 99 – Attn: Employee Benefit Department
- By fax to: 425-427-3069
- By US Mail to: Costco Wholesale, PO Box 34195, Seattle, WA 98124-1195
ATTN: Employee Benefit Department.
