Prescription Drug Quantity Limit Changes
To ensure the safety of our employees and covered family members, the medications listed below will be subject to quantity limits effective January 1, 2010. These drugs are easily misused and should only be prescribed for certain conditions and in certain amounts. The quantity limits are based on current medical findings, Food and Drug Administration-approved manufacturer labeling and cost.

Precertification Requirement
After January 1, 2010 a Medical Exception/Precertification Form from your doctor will be required the first time the drug is filled. This form is to confirm the diagnosis and that the quantity fits the new guidelines.
Aetna’s Precertification Department will process your precertification request within 48 hours. If approved, your medication will be covered and Aetna will let your doctor know. If denied, Aetna will notify both you and your doctor by mail.
To avoid any medication disruption the precertification requirement must be fulfilled by December 2009.
