Leave of Absence

If your health care provider takes you off work you must provide medical certification supporting your need to be off work to your manager.

If your health care provider takes you off of work for 5 or more days:

  • Provide medical certification supporting your need to be off work to your manager.
  • Request and complete the leave of absence paperwork

If you are unable to return to work on or before the day your leave expires, you must provide additional written medical certification supporting your continued need to be off work to your manager.

At your next appointment ask your health care provider  if you have any work abilities.  Provide updated work abilities to your manager after each office visit. 

If released to return to work with restrictions ask your manager if transitional duty is available.

Long Term Disability (LTD) benefits may be available, contact UNUM for details.

Salaried employees should contact UNUM for salaried continuation benefits if out of work for over 7 days.
