Long Term Care Plan

The Costco Long Term Care Plan is to designed help with the cost of care in a nursing home, assisted living facility or at home, provided certain conditions are met.

Who May Enroll

Long-term care coverage is available for:

  • You
  • Your spouse or domestic partner
  • Your parents or grandparents
  • Your spouse's or domestic partner's parents or grandparents
  • Your adult siblings (over the age of 18)
  • Your adult children (over the age of 18)

Even if you decline coverage for yourself, you may enroll your spouse or domestic partner when under age 85. Parents and grandparents should contact Unum directly and mention the Costco plan.

Plan Benefits

The Long Term Care Plan pays benefits for inpatient, outpatient and home health care services. Benefits are based on a facility benefit amount, which is the maximum monthly amount payable for inpatient care. Depending on the kind of covered care you receive, the plan pays a percentage of the facility benefit, as shown below. Benefits start after you've been receiving covered care for 90 days, as long as you:

  • Have lost two or more activities of daily living, skills like dressing and bathing or
  • Have a severe cognitive impairment that requires substantial supervision

Assisted living services
Adult day care or hospice services
Professional home care visits by a licensed home health care provider

$1,000 (facility benefit)
 $600 (60% of facility benefit)
$500 (50% of facility benefit)
$500 (50% of facility benefit)
$1,000 (facility benefit)

The plan pays benefits up to a lifetime maximum of 36 months or $36,000, whichever is more.
