Supplemental Accidental Death & Dismemberment Plan

You may add to your Basic AD&D coverage by purchasing Supplemental AD&D insurance. Supplemental AD&D benefits are paid to the named beneficiary in addition to Basic AD&D benefits.

Benefits for You

If you are covered by the Basic AD&D plan, you may also elect Supplemental AD&D coverage in increments of $50,000 up to:

  • $250,000 if you earn $25,000 or less per year
  • 10 times your base annual earnings if you earn more than $25,000 per year, up to a maximum benefit amount of $1.5 million.

Base Annual Earnings

Life and AD&D benefit amounts are based on your base annual earnings, which are:

  • For Full-time hourly employees, current hourly wage times 2,080 hours
  • For Part-time hourly employees, current hourly wage times 1.560 hours
  • For salaried employees, current annual pay

Base annual earnings do not include any extra compensation such as overtime pay or bonuses.

Benefits for Your Family

If you elect Supplemental AD&D for yourself, you may elect:

  • Family coverage -for your spouse or domestic partner only, or for your spouse or domestic partner and dependent child(ren)
  • Children's coverage -for your dependent child(ren) if you have no spouse or domestic partner

Family benefit amounts are a percentage of your benefit, as shown below:
