For FEHB Medical Plans:

Choose a medical plan.
Confirm family status, the plan you'd like to join, and the enrollment code for your area. You can find the enrollment code using our online Rate Calculator.
Complete a 2809 form, or enroll online, using one of the appropriate online tools provided by your agency.

Paper 2809 Form: Access the SF 2809 at www.opm.gov/forms/pdf_fill/sf2809.pdf, fill out the form and provide a copy to your Human Resource Office.

Enroll online using your agency's method (several sites are listed below):

Employee Express: See a list of agencies participating in Employee Express.
Department of Defense: DoD automated enrollment systems.
U. S. Postal Service: PostalEASE system or the telephone enrollment system.
Department of Energy: DOE automated systems.
Health and Human Services and Environmental Protection: MyPay.
Employees of agencies payrolled by the National Finance Center: Employee Personal Page.
