Aetna Open Access HMO Plan

Using your plan is as easy as 1 – 2 – 3.

The Aetna Open Access HMO Plan is a flexible and easy-to-use health plan. Here's what you need to know:

1 - Go Directly to an Aetna Network Health Care Professional or Facility.

You can go directly to a network health care professional or facility for covered services — you do not need a referral.

2 - Pay the Applicable Amount.

Whether you visit your PCP or another participating health care professional, you will be responsible for a copay (a flat dollar amount or a percentage of covered services).

The Summary of Benefits chart shows how you and the plan share the cost of some major covered expenses. When your share of covered expenses reaches the out-of-pocket maximum (also shown on the chart), the plan pays 100% of your covered expenses for the rest of the plan year.

3 - Take Advantage of the Plan's Resources.

Please note: If you sign up for Aetna and your doctor leaves the plan, you must select a new doctor. You cannot change health plans because your doctor or any other health care provider leaves Aetna.
