Earn $50 by completing the Simple Steps To A Healthier Life® health assessment.* The assessment includes questions about your health habits and family health history. Have your spouse complete a separate health assessment for another $50 incentive. All answers are kept secure and confidential.
After you complete your assessment, you’ll receive easy-to-understand health reports and a printable one-page health summary, which you can choose to share with your doctor. You’ll also get an action plan that’s just for you, suggesting a combination of Healthy Living Programs that may be right for you.

Scheduled worksite health screenings
Registered nurses will be available at certain worksites to get your “Numbers to Know.” The tests are completely confidential and help give you a better picture of your own health condition. The nurse will weigh and measure you, and take a sample of blood. You’ll receive a report showing your total cholesterol, high density lipoprotein (HDL), blood glucose, blood pressure, and body mass index (BMI). Knowing your numbers will help you and your doctor better understand your risk for disease and how your lifestyle choices are affecting you.

Healthyroads Health Coach
Work one-on-one over the phone with a personal health coach from Healthyroads to help you lose weight, quit smoking, or work on another area of your health. Your health coach will work with you to establish goals and help you identify ways to make them a reality. There are also various interactive eCoaching courses available at no cost. Each of these online courses qualifies as a separate incentive, up to the maximum incentive amount:

Preventive Care
Visit your primary care physician for preventive care services. You’ll be happy to know that most preventive care is covered at 100% with no copay. And you and your spouse can count each preventive service as a separate incentive, up to the maximum incentive amount.
Preventive care services eligible for an incentive award include a routine physical exam, annual gynecological physical exam, cervical cancer screening (annual Pap), colonoscopy, sigmoidoscopy, and routine or preventive screening mammogram, prenatal obstetrical care, cholesterol test, fecal occult blood test, and prostate-specific antigen (PSA) test. Catch problems early when they’re easier and less costly to treat.

Aetna Health Connections provides education, counseling, self-care information and physician support to help participants manage 34 chronic conditions.
Through Aetna Health Connections, you'll:

- Have the opportunity to get counseling by registered nurses to help you recognize and manage the signs and symptoms of your condition.

- Receive free booklets, videos, newsletters and other educational materials to help you understand your condition, use medications and special equipment, and avoid health problems.

Call the Informed Health Line at 1-800-556-1555 for confidential, 24/7, toll-free access to a registered nurse who can provide you with the latest information on a variety of health conditions and help you understand your treatment choices. Or, you can use your touch-tone phone to listen to pre-recorded health topics in English or Spanish.
Schedule an Appointment Free Worksite Health Screening
Worksite health screening events are a great way for you to assess your health. Health screenings give you the opportunity to increase awareness of your individual health needs and identify certain risk conditions.
Health screenings include the following tests:
- Cholesterol
- Glucose
- Blood Pressure
- Paper Based Body Mass Index (BMI)
You can complete the screenings in about 12-15 minutes and then sit down with a registered nurse to review your results.
